Oncology, Haematology and Palliative Medicine 3 - Transdermal and Oral Analgesia From Step 3 of WHO Analgesic Ladder For Chronic Pain

Prescribe a regular transdermal fentanyl patch and an oral drug for breakthrough pain from Step 3 of the WHO analgesic ladder, for a 64-year-old male with cancer pain who is struggling with tablet burden. This patient's pain has been stable on a total daily dose of 120mg of oral morphine sulfate in a 24-hour period. (Please note, the last dose of any modified release morphine sulfate tablets would be given as the patch is applied, then stopped). [Note: you need to create TWO separate prescriptions for this case BEFORE obtaining sign-off.]
